Alpha-lipoic acid is a sulfur-containing fatty acid that occurs naturally in the human body. The substance has been known since the 1930s. In 1957, it was first isolated and given the name alpha-lipoic acid. Alpha-lipoic acid consists of a chain of eight carbon atoms, each with two sulfur atoms attached. The two sulfur atoms are connected to each other, creating a ring structure. This ring structure makes alpha-lipoic acid soluble in both fat and water, an important property. Alpha-lipoic acid is a molecule found in the mitochondria of every cell.

Our bodies produce small amounts of R-alpha-lipoic acid. The R-form of alpha-lipoic acid is also found in small amounts in foods, such as liver, spinach, and broccoli. In food, alpha-lipoic acid is always bound to protein (specifically to the amino acid lysine). Before it can be absorbed, alpha-lipoic acid must first be separated from lysine. In supplements, alpha-lipoic acid is available in its free form (as a mixture of the R/S form or as a pure R-form).
R-form most effective
There are two forms of alpha-lipoic acid: R-alpha-lipoic acid (the natural, endogenous form) and S-alpha-lipoic acid (the synthetic form). Most products on the market are a mixture of these two. S-alpha-lipoic acid does not occur naturally and is foreign to the body, potentially making the R-form less effective. Therefore, a supplement consisting entirely of the natural R-form is preferred. The problem with R-alpha-lipoic acid, however, is that it is quite unstable and temperature-sensitive. Manufacturer GeroNova has therefore developed bio-enhanced sodium R-lipoate, in which R-alpha-lipoic acid is bound to the mineral sodium. This makes the R-form much more stable and has optimal bioavailability. As a result, lower doses of sodium R-lipoate are sufficient than with a supplement containing the mixture (usually 50% R- and 50% S-alpha-lipoic acid).
